This part was removed from a Cummins Big Cam engine. This is a: Jake Brake Head Rocker Arm Plate - Photo shows 3 pieces but sale is only for 1. Please note that this has some damage on one of the rockers. This engine was removed from a 1996 Peterbilt that broke a rod, and damaged the block. I am not sure of the exact model number but I have included photos of the engine before it was disassembled. I have been told that this is a Big Cam 400, but cannot confirm.
